Covid-19 Registration Form

One time registration to comply with Health and Safety

As required by law, we request that you complete this form prior to returning to classes in September. We will be using an app for check in and out which will make the process easier, faster and more effective and saves you having to complete a new form on each visit!

You only need to complete this form once.

ALL EXISTING AND NEW PUPILS AND THEIR PARENTS OR ANY RELATIVE THAT MAY NEED TO ENTER OUR PREMISES MUST COMPLETE THIS FORM.

You will then be redirected to a page detailing our fast check in process via the app EVEPASS.

Simply follow the step by step guide.

Then check in directly using the app when you arrive at class. (It is simply complying with government regulations to keep all of us safe)
